Silent GERD with microaspiration is a real and common cause of recurrent lower lobe pneumonias, particularly in patients who sleep flat, consume alcohol regularly, or have a hiatus hernia. At Pulmovista Clinics, I screen every recurrent infection patient for GERD — particularly those with lower lobe predominance, nocturnal or early-morning infections, and those without any obvious respiratory predisposing factor.
